To My Valued Patients,

I am writing to inform you that effective Monday, September 16, 2024, I will be closing my practice and starting my next chapter. Unfortunately, due to multiple extraneous reasons, my business will be filing for bankruptcy protection.  I will no longer be available to take care of your foot needs after that date.  I have enjoyed my career at Bolingbrook Foot and Ankle Center, and it is hard to say goodbye to you.  It has been a privilege to have been your podiatrist over the years.  I wish you and your family the best of health in the future. 

Should you require further foot care, I can recommend Dr. Joe George (815-741-6900) or Dr. Nancy Jagodzinski (630-778-7670). You may also use your insurance company’s provider directory or check with your hospital’s physician referral service. 

If you require a copy of your medical records, I have attached a records release authorization form for your convenience. In accordance with Illinois state law, the fee for medical records will be as follows:

If you choose to pick up a copy of your medical records, please give us advance notice and do so no later than Monday, September 30^th.  After September 30^th your records will be held off site and can be made available by calling 630-474-8696.
